I understand that “foster pet(s)” means the animal(s) described above, who is/are the subject of this foster agreement.(Required)

I agree to provide a foster home for the above-named foster pet(s).(Required)

I understand and agree that the foster pet(s) is/are the property of Animal Rescue of the Rockies. Animal Rescue of the Rockies retains full ownership over the foster pet(s) during the foster period, and while the foster pet(s) is/are in my possession. If I fail to return the foster pet to Animal Rescue of the Rockies at the end of the foster period, Animal Rescue of the Rockies will notify me by phone of the reclamation and ask that the animal(s) be returned. Return of the foster pet to Animal Rescue of the Rockies must occur within 24 hours of notice of reclamation of the animal. Return of the foster pet(s) to Animal Rescue of the Rockies is at Animal Rescue of the Rockies’ sole discretion. If the foster pet(s) is/are in such a circumstance to endanger their health, well-being, or life, Animal Rescue of the Rockies will designate a volunteer to recover the pet immediately or call animal control authorities to recover the animal on behalf of Animal Rescue of the Rockies. Animal Rescue of the Rockies has sole and final discretion regarding whether the foster pet(s) will be returned to me.(Required)

I shall keep all Animal Rescue of the Rockies identification tags on the foster pet for the entirety of the foster period.(Required)

I agree that the foster pet’s microchip will remain with Animal Rescue of the Rockies’ contact information (address, phone number, and email address). I agree not to remove or alter Animal Rescue of the Rockies’ contact information from the foster pet’s microchip. In the event the microchip stops working (backs out after implantation, microchip cannot be found with a microchip scanner, etc.) I agree to notify Animal Rescue of the Rockies without delay so that a new microchip can be implanted.(Required)

In the event foster pet becomes lost; I will immediately notify Animal Rescue of the Rockies. I will immediately notify Animal Rescue of the Rockies of any change of contact information (address, phone number(s), or email address). I authorize any individual or agency to release the foster pet back to Animal Rescue of the Rockies if the pet is found abandoned, running at large, or is brought to a shelter facility for any reason. I agree that, at Animal Rescue of the Rockies’ sole discretion, Animal Rescue of the Rockies may retain possession of the pet and terminate the foster period if the pet becomes lost through a breach of this agreement.(Required)

I acknowledge that Animal Rescue of the Rockies has disclosed in writing any existing illnesses, injuries, or behavioral issues to me regarding this foster pet.(Required)

I understand and attest that I will not leave the foster pet unattended with small children(Required)

In the event that an animal shows signs of illness, injury, or behavioral issues, I will notify Animal Rescue of the Rockies immediately. Animal Rescue of the Rockies cannot guarantee the health of any foster pet and makes no such guarantee. Animal Rescue of the Rockies is not responsible for any illness or injury caused to any current pets in the foster household by the foster pet.(Required)

All veterinary care for the foster pet(s) must be at a pre-approved veterinary clinic. I agree to obtain Animal Rescue of the Rockies approval prior to any veterinary care for the foster pet. I understand and agree that Animal Rescue of the Rockies will only be responsible for the cost of care at a pre-approved veterinary clinic or after a prior arrangement with Animal Rescue of the Rockies for a professional discount with another veterinary clinic. If veterinary care is provided outside of a pre-approved veterinary clinic, I understand I will be responsible for payment of the cost, unless otherwise agreed to in writing by Animal Rescue of the Rockies. I understand that in the event of any out-of-pocket costs to me, these costs are not proof of ownership of the foster pet.(Required)

I agree to bring my foster pet(s) to as many adoption events as possible until the foster pet(s) is/are adopted. I will notify the adoption event coordinator if I need help transporting the foster pet to or from an event.(Required)

I shall provide the foster pet(s) with a secure area, including shelter from the elements. The foster pet will not be kept tied or chained.(Required)

The foster pet(s) will not remain in cars unattended or unsupervised for extended periods of time. The foster pet(s) will be transported in secure crates to avoid potential injuries or escape from vehicles.(Required)

I agree not to leave the foster pet(s) overnight in the care of anyone other than myself.(Required)

If the foster pet(s) requires behavioral training, I will accommodate Animal Rescue of the Rockies in scheduling and coordinating training sessions. I will assist in positive training for basic household manners.(Required)

If the foster pet(s) is a dog(s), I agree to use a correctly fitted harness or martingale collar at all times when the foster pet(s) is not in a secure and enclosed area within the foster home. I will never allow the dog to roam freely at any time, even for potty breaks. I understand that “roam freely” means outside of a securely fenced area which the foster pet cannot escape.(Required)

If the foster pet(s) is a dog(s) that will go to work with me, I agree to ensure the dog(s) behaves appropriately in an office setting.(Required)

I agree that any and all Animal Rescue of the Rockies foster cats and kittens must remain inside at all times. I understand that if the foster cat or kitten is allowed to roam outside it will be grounds for return of the foster animal to Animal Rescue of the Rockies.(Required)

I understand that only two (2) foster pets may be fostered at the same time by the same foster, unless an exception is approved by Animal Rescue of the Rockies.(Required)

If an exception to foster more than 2 pets is requested, please explain.(Required)

I understand that Animal Rescue of the Rockies will conduct home checks prior to the foster pet(s) being placed in my care. I understand that Animal Rescue of the Rockies, at its sole discretion, may perform home checks at any time during the foster period with advance notice.(Required)

I have read and familiarized myself with the foster pet's file. I understand the foster pet's specific needs and have been educated on the proper ways of working with this foster pet's special needs, including any illnesses, injuries, or behavior issues.(Required)

I will ensure compliance with all applicable local, state, or federal ordinances or statutes regarding possession of the foster pet. If I am charged with violating any such ordinance or statute while in possession of foster pet, I understand Animal Rescue of the Rockies is not responsible for obtaining legal counsel for me or for paying any fines or penalties assessed(Required)

I affirm that no member of my household has been convicted or charged with any animal welfare law violation such as hoarding, unsanitary conditions, neglect, cruelty, abandonment, etc. I affirm that no member of my household has been convicted or charged with any domestic battery or sex crime.(Required)

If I wish to cease fostering with Animal Rescue of the Rockies at any time, I will notify Animal Rescue of the Rockies immediately for the foster pet's return to Animal Rescue of the Rockies. I agree that I am then bound to return the foster pet to Animal Rescue of the Rockies and may not transfer ownership or possession to any other party. If I attempt to wrongfully transfer ownership or possession of the animal, such an act will be determined to be a breach of this contract, subject to the terms and conditions described therein. Any decision by Animal Rescue of the Rockies or me to end the foster relationship shall not be construed as a waiver of any other portion of this agreement.(Required)

I hereby acknowledge the foster pet(s) shall not be sold, given to, or adopted to anyone else, nor shall there be any transfer of ownership to any firm, corporation, or any other individuals, humane association, animal shelter, animal control facility, medical/experimental laboratory, or any other organization, or as part of any criminal plea agreement. The foster pet(s) must be returned to Animal Rescue of the Rockies if, for any reason, I can no longer care for it. Violation of this agreement will result in court costs and attorney fees payable by me if necessary for the Animal Rescue of the Rockies to redeem the foster pet(s), as well as damages for the monetary value of the foster pet(s) as determined by this contract.(Required)

I understand and agree I am the party responsible for the foster pet(s) actions while the animal is in my care. I agree to foster the foster pet(s) at my own risk. I agree to assume full responsibility for the conduct of the foster pet(s) while the animal is in my care. I further agree to release, indemnify, and hold harmless Animal Rescue of the Rockies and Animal Rescue of the Rockies representatives, including its Board of Directors and volunteers, and to indemnify such organizations or individuals from any damages, costs, or injuries resulting from liability, known or unknown, anticipated or unanticipated, as a result of the foster pet(s) conduct while the foster pet is in my possession. I understand and agree that I am solely responsible for damage, illnesses, or diseases and/or injuries caused by the foster pet(s). I understand that if the foster pet(s) bites or injures any human or other animal, I will hold harmless and indemnify Animal Rescue of the Rockies from any claim or suit filed by anyone as a result of such an incident.(Required)

I acknowledge and agree to notify Animal Rescue of the Rockies of any criminal complaint, summons, indictment or other violation of law brought against me involving the foster pet. I further agree to notify any relevant animal control facility, municipality, county, state government, district attorney, prosecutor, or other individual, organization, or enforcement agency, holding, detaining, or impounding the foster pet of my contractual obligations with Animal Rescue of the Rockies that preclude surrender of the foster pet. I agree to cooperate with Animal Rescue of the Rockies and law enforcement to ensure the return of the foster pet to Animal Rescue of the Rockies if in the best interest of the foster pet and public health and safety. I will not offer to surrender the foster pet to law enforcement for permanent forfeiture or voluntary euthanasia request and will instead direct law enforcement to attempt to coordinate return of the foster pet to Animal Rescue of the Rockies.(Required)

Any violation of the terms of this agreement shall be grounds for immediate termination of the foster placement, and the foster pet shall be immediately surrendered to Animal Rescue of the Rockies. The foster placement may be terminated with or without cause at the sole discretion of Animal Rescue of the Rockies.(Required)

I understand that if I am found to have materially misrepresented any fact or portion of my foster application, this contract is materially breached, and Animal Rescue of the Rockies may demand the return of the foster pet(s). I understand that if I am found to have materially misrepresented any fact or portion of my foster application, I agree to pay any and all costs and fees associated with the return of the foster pet(s) to Animal Rescue of the Rockies, including attorney fees.(Required)
